Long Term Conditions Prevention – Hypertension

The African Caribbean Care Group (ACCG) aims to enhance community health by increasing the awareness of hypertension and promoting proactive blood pressure management. Recognising that high blood pressure often goes undetected, we will implement a multi-faceted approach combining opportunistic interventions with targeted efforts focused on individuals at higher risk.

Our goal is to support individuals from African Caribbean and marginalised communities within North Trafford to increase their awareness of hypertension risks and providing practical advice to reduce its impact on life expectancy through a holistic community support package.

We will promote the project at community events, workshops, and through social media. Brief advice sessions will be offered, focusing on hypertension, healthier lifestyles, and practical tips for blood pressure management.

During these sessions, participants will be offered blood pressure checks, and anyone identified as having elevated blood pressure or being at risk will be signposted to GPs and other relevant health services.

The African Caribbean Care Group is proud to announce that we have received funding from Independent Age to support women over the age of 65 from racially minoritized communities.

  • Increase Knowledge of Financial Support: We provide comprehensive information about the financial assistance available to ensure that every woman is aware of the support she can access.
  • Boost Confidence in Accessing Support: Through our programs, we aim to empower women to feel confident and capable of seeking the help they need.
  • Offer One-to-One Support and Guidance: Personalised assistance is available to help women claim the benefits they are entitled to, ensuring they receive the support they deserve.
  • Expand Support Reach: We are dedicated to increasing the number of women from racially minoritized groups who receive our support, making a positive impact on their lives.
